Billing & Payment Procedures
Our billing process is designed for transparency and ease. We handle insurance claims directly and will clearly communicate any patient responsibility.
Our Standard Process
- 1. We submit claims to your insurance provider on your behalf.
- 2. You will receive an Explanation of Benefits (EOB) from your insurer.
- 3. We send a statement for any co-pays, deductibles, or co-insurance as determined by your plan.
- 4. Payment is due upon receipt of our statement.

Print the forms at home. Please fill them out completely and legibly using a black pen.
2. Bring to Your Appointment
